Health & Wellbeing Lead Practitioner – Supported Living

We have a vacancy for 2 Full Time Lead Practitioners, based in Edinburgh for a fixed term of 12 months.

This is an exciting opportunity to make a real difference to people’s lives.

You will:

  • Lead a small number of teams delivering person centred support to a wide range of people living in their own homes.
  • Work alongside people we support to plan and create great support plans focused on what really matters to them.
  • Support teams to think creatively about all aspects of working with the people we work alongside so they have great lives.
  • Be able to provide teams with strong leadership which motivates and delivers your passion for really high standards of support. You’ll be able to provide effective coaching and also to give strong direction when required.
  • Set up new services and support the creative use of personal budgets to support people to have great lives.
  • Provide individual supervision to your staff, facilitate regular Practice Development Group meetings, and set clear objectives for all your staff.
  • Have 3 years’ experience in supporting people with disabilities, working in a person centred way, facilitating groups and ideally managing teams, together with excellent organisational and IT skills.

Health and Wellbeing Practitioner

We are looking for a Health and Wellbeing Practitioner to support the Wellbeing Service across Midlothian. The Wellbeing service offers person centred, holistic support for people with or at higher risk of long term conditions. The Health and Wellbeing practitioners work in small teams based across GP practices in Midlothian to support a holistic supported self-management service for people with long term condition(s) and support the development of peer support within the service.

The main purpose of this role would be to:

• Use a strengths based personal outcomes approach to help people referred to the service identify and access appropriate support for them to self-manage.

• Manage a caseload of people referred to the service

• As appropriate provide one-to-one and group based support for people using the Wellbeing service

• To appropriately support people to access other services that would support their self-management journey.

• To provide an outreach service for the Health and Wellbeing team to engage hard to reach clients with the service

• To support volunteers/paid peer supporters who have lived experience of long term conditions into volunteer/ peer roles to support the Wellbeing team

• To monitor and report on own work as per agreed monitoring tools and to highlight areas of concern and areas of success

• To contribute to the organisational strategy and associated key performance indicators by participating in team meetings as required

• Create meaningful and useful cross-referral links with local leaders and contacts in health, local authority, voluntary and community organisations.

• To support the Wellbeing team’s work to provide timely access to the service in Midlothian.

Working as a Personal Assistant (PA) means making a difference in people’s lives by supporting them to live independently in their own homes and in their community.

What does a PA do?

  • PAs plan and provide support and direct care to people. This will be personalized according to the needs and preferences of the supported person

Personal Assistants Wellbeing Support Team

We are also seeking 2 experienced Personal Assistants (Support Workers) to join the Wellbeing Support Team.

  • The Wellbeing Support Team works alongside established teams within supported living services to provide a back-up service through workforce co-ordination; ensuring that our values are embedded and that continuity of support is provided.
  • This role offers an opportunity to develop leadership and coaching skills with potential to progress within the organisation.
  • If you have experience of working with people in a person-centred way, feel you can positively contribute to a self-organised team and can demonstrate excellent interpersonal and communication skills, this could be an ideal role for you.

A certain amount of flexibility is required for this post as the shift patterns vary. There will be a requirement to be ‘on call’ at times, and an allowance is payable for this. A driving licence is desirable for this role.
